Neonatal Surgery

Abstract
Neonatal surgery is one of the youngest branches of surgery, and is the most rapidly developing part of pediatric surgery. Pediatric surgery itself is a very young specialty and has only become established in most countries. It is, therefore, not surprising that the organization and support of neonatal surgical service as well as pediatric surgery is, as yet, in its infancy and that much of it is still at a stage of development in Korea.
For the purpose of better understanding and revealing the problems in the management of neonatal surgical patients and their diseases, analysis of 104 neonatal surgical patient who admitted Dept. of Surgery, Seoul national University during ten year period;1963.4-¢¥1973.3) were made: Following conclusions¢¥ and problems appeared:
1. There were 78 boy and 26 girl.
2. About 40% of the patients were admitted during their first week of life, but there were 34% of patient who admitted during last week of their neonatal period.
3. Variety of diseases affected this tiny baby. Major malformation were ano-rectal disease. Tracheo-esophageal fistula, pyloric stenosis, and G-U anomalies were less common compare to incidences appeared in English literatures.
4. Total number of neonatal patients were too small for the size of population of Seoul District. Same figure appeared on other investigators report too. Possible explanations were: a. delayed diagnosis
b. parental negligence, shame for having crippled baby
c. commitment by so called occasional pediatric surgeon in untrained, unequipped establishment d. poor financial situation of the family
5. There were distinct difference in fatality and complication rate between groups who received their surgery by amateur surgeon (usually chief surgical resident) or experienced pediatric surgeon.
6. There are much to be improved in both immediate survival and long-term functional results.
7. Over all surgical mortality rate was 13.8%. (17.6% for first 5 year dropped to 10. 1% for last 5 year)
